The vessel will be considered for berthing only after submitting the pre-arrival to Port.

• ISPS Form (New)
• Last port Clearance/Last Port Immigration clearance copy (Next Port should be “Fujairah”)
• Certificate Check List
• Fujairah Immigration Crew List – On attached Format – Pls send both PDF & Word/Editable format
• FAL Forms (1 to 7) – Attached – Pls. fill up the all FAL Forms (01- 07) as it is Fujairah Port will not accept any other format of FAL Forms
• NTM Compliance of 148v5 & 225
• NTM Compliance of 271 & 274
• Maritime Declaration Of health from (MDH Annex 8)
• Master Declaration for No arms/Ammunitions

The Files Available In The Download Link Needs To Be Filled. All Files Need To Be Filled And Submitted To Saqr Port Atleast 72 Hrs Prior Arrival Of The Vessel.

• Last port Clearance/Last Port Immigration clearance copy (Next Port should be “Mina Saqr”)
• Mina-Saqr Immigration Crew List – On attached Format – Pls send both PDF & Word/Editable format
• FAL Forms (1 to 7) – Attached – Pls. fill up the all FAL Forms (01- 07) as it is without changing the format and send back each form separately with sign & stamp. Port will not accept any other format of FAL Forms.
